Under Eye Bags vs. Dark Circles: Why They're Different—and Why the Right Diagnosis Matters
It’s one of the most common concerns I hear during consultations.
Many people use the terms under-eye bags and dark circles interchangeably, assuming they’re the same problem. In reality, they often have completely different causes—and treating them incorrectly can waste time, money, and lead to disappointing results.
One of the biggest misconceptions in aesthetics is believing there’s a single treatment that fixes every under-eye concern. Social media often suggests that everyone needs filler, while others claim PRF is the answer for everyone. The truth is much more nuanced.
As a physician, I believe successful treatment starts with understanding why your under-eyes look the way they do before deciding how to treat them.

The Four Most Common Causes of an "Aged" Under Eye
1. Volume Loss (The Tear Trough)
As we age, we don't simply develop wrinkles—we lose structural support. The bones of the face gradually remodel, fat pads shrink and shift, and collagen production slows. The result is a hollow beneath the eyes known as the tear trough. This hollow creates a shadow that many people mistake for dark pigmentation. The skin may actually be a normal color, but the way light falls into the hollow makes the area appear darker, which is why makeup often doesn't cover it.
Key Characteristics: Structural bone remodeling · Hollow shadows mistaken for pigment · Shadowing resistant to makeup
2. True Pigmentation
Some patients genuinely have darker skin beneath the eyes. Unlike hollowness, pigmentation doesn't improve simply by replacing lost volume. Instead, treatment focuses on improving skin quality, reducing pigment, and protecting the area from further damage.
Common Causes: Genetics · Sun damage · Post-inflammatory pigmentation · Chronic rubbing from allergies · Specific skin types
3. Puffy Under Eye Bags
This is very different from hollowness. Under-eye bags are usually caused by fat pads beneath the eyes becoming more prominent as supporting tissues weaken with age. One important point many patients don't realize: adding filler to a patient with significant eye bags can actually make the area appear heavier, which is why an accurate diagnosis matters.
Contributing Factors: Prominent fat pads · Genetics · Salt intake · Allergies · Fluid retention · Lack of sleep
4. Thin Skin
The lower eyelid contains some of the thinnest skin on the body. As collagen decreases over time, tiny blood vessels become more visible, producing blue, purple, or reddish discoloration. Patients often think they need more volume when what they actually need is healthier, thicker skin.
Best addressed by: Improving skin health · Collagen stimulation · Dermal thickening rather than volume replacement
WHY SOCIAL MEDIA GETS IT WRONG
Single-treatment reveal videos have become incredibly popular online. Someone receives filler. Another receives PRF. Another has laser treatment. The video shows an impressive before and after.
What you don’t see is whether that treatment was appropriate for your anatomy. Two patients can walk into my office with nearly identical complaints and require completely different treatment plans.
That’s because I treat the underlying cause—not simply the symptom.
So Which Treatment Is Best?
PRF — Platelet-Rich Fibrin
PRF is one of my favorite regenerative treatments for the under-eye area because it uses your own blood-derived growth factors to stimulate collagen production and improve skin quality over time. Patients with thin skin or early signs of aging often benefit from PRF because improvements continue to develop gradually as collagen remodels.
Best for: Thin skin · Early signs of aging · Gradual, natural-looking improvement
PRF Gel
PRF Gel offers many of the regenerative benefits of PRF while also providing subtle structural support. For carefully selected patients who want a natural approach to improving mild hollowness, it can be an excellent alternative to traditional filler.
Best for: Mild hollowness · Natural alternative to traditional filler · Carefully selected candidates
Hyaluronic Acid Fillers
Under-eye filler can produce beautiful results—but only in the right patient. The tear trough is one of the most technically demanding areas to inject. Proper patient selection is just as important as injection technique. Not everyone with dark circles should receive filler — in fact, one of the most valuable consultations I provide is explaining when filler shouldn't be performed.
Best for: True structural hollowness · Carefully selected anatomy · Confirmed filler candidates
Skin Tightening Treatments
When loose skin contributes to an aged appearance, collagen-stimulating treatments can improve firmness and elasticity. Depending on your anatomy, technologies such as Exion, Morpheus8, or other collagen-stimulating devices may be recommended to improve skin quality without surgery.
Best for: Loose or lax under-eye skin · Improving firmness without surgery
Fractional CO₂ Laser
For patients with fine lines, crepey skin, and certain types of pigmentation, fractional CO₂ laser resurfacing can significantly improve texture while stimulating new collagen production.
Best for: Fine lines · Crepey texture · Certain types of pigmentation
Lower Blepharoplasty
Sometimes the best treatment isn't non-surgical. If prominent fat pads or excess skin are the primary issue, lower eyelid surgery may provide the most predictable and long-lasting improvement. One of the responsibilities of an experienced aesthetic physician is recognizing when surgery—not another syringe—is the better recommendation.
Best for: Prominent fat pads · Excess skin · Cases where non-surgical options fall short
Why Combination Therapy Often Produces the Best Results
Many patients don’t have just one problem. They may have mild volume loss, thin skin, pigmentation, and early skin laxity all at once. Trying to solve all of these with a single treatment usually leads to compromise.
Instead, combination therapy often creates the most natural-looking outcome. One patient may benefit from PRF to improve skin quality, laser resurfacing to address fine lines, and a conservative amount of filler to restore lost support. Another patient may be better served with PRF Gel and collagen-stimulating treatments alone. Every plan should be individualized.
My Philosophy
A treatment isn’t “good” or “bad.” It’s simply either appropriate or inappropriate for a particular patient. That’s why I don’t believe in one-size-fits-all aesthetics.
Your anatomy. Your skin quality. Your age. Your previous treatments. Your goals. All of these influence what I recommend. Sometimes that means filler. Sometimes it means PRF. Sometimes it means surgery. And sometimes the best recommendation is to do nothing at all.

Frequently Asked Questions
1. Can under-eye bags be treated without surgery?
Mild to moderate concerns often improve with regenerative treatments, collagen stimulation, or skin tightening, but significant fat prolapse may require surgery for the most predictable result.
2. Are dark circles always caused by lack of sleep?
No. Genetics, pigmentation, thin skin, volume loss, allergies, and aging are all common causes.
3. Is PRF better than filler?
Not necessarily. They treat different problems. The best option depends on the underlying cause of your under-eye concern.
4. Can filler make eye bags worse?
Yes. In patients with significant puffiness or fat prolapse, filler can sometimes accentuate fullness rather than improve it. This is why proper patient selection is essential
